Keyword "May." If you're playing Relentless Rats, you don't have to cast them all.

I made a really good 4 color deck with this and cascade. I was able to get 44 life. Kill 8 of my opponent's creatures, and have 4x +4/+0 Enlisted Wurm. 4x +4/+0 Bloodbraid Elf, 4x +4/+0 Kitchen Finks, 4x +4/+0 Murderous Redcap, and 4x +4/+0 Boggart Ram-Gang all on turn 6.

Typical R.Rats gaming situation:
Rat Player (here on out RP): I play Thrumming stone!
Opponent: Oh yay.
RP: I now play Relentless Rats! And every single one of my 40 copies!
Opp: OH SHI-- Day of Judgment.
RP: FFFFFFFFFFFFFFFFFFFFFFFFFFFF---

OK, so this and Relentless Rats is a given.
But it goes oh so lovely with Door of Destinies, since you still cast the card.
So now, when you get 30 rats out with this, they're not 31/31s. They're 61/61s.
Tasty.

Can you activate the ripple ability by casting a spell from the graveyard with Flashback? I'd go straight for Roar of the Wurm, myself. Or maybe go with the Rats and throw in some Folk Medicines?

The best way to use is with Beacons. Like Beacon of Unrest (which could also return the Thrumming stone back to play in case it gets destroyed) or Beacon of Destruction. Then you should thin your library as quickly as possible. The fewer cards there left, the higher the chance the beacons will turn up.
I got a blue-black deck around that, using hand disruption and bounce to buy time, and many small cantrips and card drawers to thin the library. Since they ripple as well, this works very quickly. No tournament deck, but still fun.
