Feels a bit underpowered for 4. I don't know if I would make room for this guy in an infect deck, decent but certainly not game breaking.

GIBLIC, but nice flavor. For those who don't know, this is a Herxheimer reaction. When exposed to the first generation of antibiotics, Treponema (the bacterium that causes syphilis) releases all its toxins in its death throes.

I think people are underestimating this guy.
It's either 2 poison counters,
Or killing a 3 toughness or lower creature, plus a poison counter (by T5 you better have a counter on your opponent).
Honestly, even if it isn't as aggro as most competitive infect decks,
This guy is a pretty solid addition to Limited.

Trades with x/3's and delivers a Shock on death? Not bad.
EDIT: By Shock I mean, it does 1 poison counter worth of damage to a player which is equivalent to a Shock.

Use with Tezzeret, Agent of Bolas.
"Let's see, I'll make Core Prowler a 5/5... oh, you Doom Blade him? Well I guess Tezzeret gets more loyalty and you get more poison. Boo hoo."

Brutal card. 2/2 Infect is worth about 3. Proliferate is worth about 2. Phyrexia loves sac-ing artifacts.
Zero complaints. Hilariously effective in almost any infect deck.
