I think in standard I'd rather play this over clan defiance... leaves my mana open for bloodrush

I love this card. Being able to wipe out a big creature for 2 mana is very nice. Sure, he can't target players and he is sorcery speed, but he can nail annoying creatures for a relatively low cost, and he scales well in ramp decks.
Definitely better than it looks.

Can this card damage the actual player or creature cards only? Also the land total, can you include the two mana used to cast Ground Assault or are they subtracted?

Its effect (usually straight up killing a dude) is well worth splashing one of its colours for in limited. What amounts to a hard-to-cast sorcery speed doom blade doesn't seem very good for constructed though.

I think the rating is appropriate, only because you need 5 lands for it to be mana-efficient.
Two mana is a way, way worse Shock, three mana is a restrictive Searing Spear, and four mana is a non-Overload, non-mono-red Mizzium Mortars. You have to be in the late-game before this ever matters. Drawing it any earlier is a waste.
