Ok in limited, sucks otherwise. The main problem is the mana cost of its activated ability. Normally, you want to sacrifice such creatures in respond to removal. "You want to shock my guy? I sac him in response". But it's very unlikely that a black deck has three mana around at all times for that.
It's also just too expensive in general. A raise dead does the same and costs one mana instead of a total of five, that you have to pay here for the effect.
I'd always play cadavre imp over this. Only two power, but it does the whole job for three mana and can keep attacking, with evasion, while you already got the card back from the gy. Gravedigger is superior too. Costs two more, but is in total still cheaper than this guy, and you always get the creature card back (unlike this guy, which, when killed when you have no three mana around, doesn't give you anything). And it's also a zombie, so it gets buffed from all sorts of zombie cards.

I like how the flavor text helps to describe why you have to sacrifice him to get the effect. That being said, Entomber Exarch is probably better and more abusable for this type of effect however, a 2/1 for 1 with upside is always going to be a fine man in limited and this upside has the potential to be very powerful. There are also cards that do stuff when creatures die like Grave Pact and Harvester of Souls that have good synergy with this.
