This is an interesting example of flavor in action, however the second ability makes me really wary of playing this creature...

Of course, it will sound cliche, but, dies to Terror.
And for the reason alone, I would not use this card.

Something like,
Player: I play Personal Incarnation.

Opponent: "K, let me read what it does. Oh, okay, I let it resolve. Terror. Take 10 Damage."

@SquirePath: So you're telling me you only play Black and Artifact creatures due to one card?

Personally, I prefer Avatar of Me. I find it to be much more efficient and fun.
