You forgot about Carnophage and Phyrexian Negator!

Regarding the card itself, it can be useful with other Black-costed cards:
Vampiric Link is useful since it does not only offset the damage it deals to you, but also provides two life each turn,
and Bloodchief Ascention, since it will trigger each turn and eventually drain your opponent's life whenever one of his/her permanents gets destroyed once it has three or more counters.
With that much lifegain stuff Sanguine Bond might even find use, too.

This card can be deadlier than it looks. It could easily kill your opponent. It will put them on a 10 turn clock minimum if they don't get rid of it or gain life. If you have Drudge Skeletons or a Steel Wall, you could chump block that thing all day.

This plus a Pacifism is a nasty early game combo. You could also try Crippling Blight for maximum insult. Your opponent may even use their own removal to get rid of it.

However, you are, of course, giving them a creature. Gives them something to enchant/buff.

Fits black nice too, desperately trying to win at the cost of their resources.

Cards like this, Dross Harvester, Dark Confidant, Flesh Reaver and Hatred define "true" Suicide Black.

How is this suicide black? your opponent controls the creature. If anything, it forces them to play suicide black. Still a very good card, and i really like the idea of this combined with Bloodchief Ascension. You know what else loves the manipulation of life? The Orzhov Guild! Use pillory of the sleepless to lock it down and have them lose 3 life a turn automatically, or just siphon life from them via other methods to keep yourself ahead in life.

This + Stab Wound. I reckon your opponent will try to get rid of it as soon as possible in that case.

Souls of the Faultless makes the opponent have to think fast. Attacking into the Souls would only benefit you and hurt them even more! There are also a ton of great suggestions in the other comments.

This and Stab Wound together feels right flavorwise. Either they are a double agent and about to die, so they do much more damage, not caring if they get caught, or they go to the other side and use the knife that caused the stab wound against their enemies. Wonderful.

Also, coincidentally, was designing a card for fun the other day that did a similar thing. It was a bit more complicated but same idea. And I called it sleeper agent. Then I found this. Spooky.
