This and Necropotence are probably the two most hardcore black cards I know of. When you see one, the game is probably going to end pretty soon. They both cost triple black. It must mean something.

Russian Roulette anyone? A fun but weird combo would be to play this when you have Killer Instinct in play and leave four of the following epic creatures in your library:
Emrakul, the Aeons Torn
Ulamog, the Infinite Gyre
Kozilek, Butcher of Truth
Progenitus
Darksteel Colossus
Blightsteel Colossus
And the fifth card you ask? Phage the Untouchable
You may set the order of your first "bullet" but your deck will soon be shuffled as each epic card is shuffled back in.
If you want to give your opponent some of the fun have Lurking Predators in play too. A card to prevent your card draw would be good too e.g. Necropotence or Possessed Portal. Temporal Aperture could be useful
to benefit from the eldrazi "when cast" abilities too.

This card is already amazing, but just imagine what it will be like when the MIRACLE cards show up in Avacyn Restored.
