Science fail. In the world of MAGIC. These could be genetically altered puffers, who also blow bubbles and enjoy Saturday morning cartoon shows in addition to giving random folks fun bonuses.

It's not that bad for Commander I think. The commander damage rule is very relevant there (if you are dealt 21 or more damage from a single commander, you lose), so the puffer extract can be a big help in getting there.
It also can be used on indestructible or regenerating creatures without much of a drawback. Or you can tap it just for 0 mana, to kill creatures an opponent would want to steal from you, or that you stole for a turn, or to trigger "whenever this/a creature dies" abilities. Or when your commander is unable to do something because of an aura or whatever, you can use this to kill him and replay him. And so on.

Maybe overcosted by today's standards, but not a useless card. Playable in token decks, swing with you army then enrage the one that gets through. Or just use it as an all or nothing finisher.

This looks like it'd give a late game Spark Elemental a reason to exist. Assuming you only have the five lands you needed to cast the Puffer Extract, then you can cast the elemental and give it +4/+4, making it a 7/5 hasted trampler that was going to die at end of turn anyway.
