This is a fair card. Putting a permament -3/-3 on a creature is much more dangerous than people think.
It is also fairly costed, compared to incinerate, too. Could possibly go well in an infect deck.

If you have the black, skinrender is a better choice. You lose instant speed and face-slapping, but trading that and 1 mana for a 3/3 body is worth it.

In EDH or in a deck that plans on burning creatures instead of players mostly, this can actually be better than Lightning Bolt. Puncture Blast is one of the few red burn spells which works great when used on a big creature. For example, it can turn a Serra Angel into an insignificant 1/1, while Lightning Bolt won't do anything against it unless it was already dealt damage. Still, the bolt is better in most decks.

This card does have the benefit of crippling things higher then 3 toughness. Personally I dont like my burn spells over the 2 mana threshhold.
