@DritzD27: Usually when you have two triggers that would occur at the same time, and you control both of them (n this case both trigger at the Declare Attackers step), you may choose which order they go onto the stack. The best case for the Trepanation Blade would be to stack the Twin trigger first, then the TB trigger so that when they resolve you get the power boost for Gemini and the Twin will then appear with the higher power.
@DariusIV: If you have declared the Gemini Engine as an attacker, the "create a twin token" ability will have been triggered and will exist on the stack. Abilities on the stack can exist independently of the source that created them and aren't countered if the source of that ability is destroyed, exiled or bounced. (Magic Comprehensive Rules; Abilities Section 112.7a)
So if the Gemini Engine is in play and declared as an attacker, the Twin token will come into play, attacking, even if Gemini disappears.

Woah this + exalted. If you put his triggered ability on the stack "first", he will get beefed from exalted and then make the token.

If only the token had also had all abilities of Gemini Engine, then it would've been amazingly powerful. Still nice that you can buff the Engine and then the token will have the same power and toughness.

This card is like Stangg's twin. Great with any pump spell(and he can be played in the colors that have them). Also good with Intruder Alarm, effectively giving him vigilance.

Id usually rather have doublestrike and, if I can have it, trample. Unfortunately boros keyrune is the only "artifact creature" with doublestrike.
The problem with this is you really have to put work into keeping it alive. Its 6 mana with only 4 toughness. If it is blocked and killed the token really doesnt count for much. At that point I would usually rather have an unblockable creature, which is often far cheaper.
I feel like this just tries to be good at temporary token generation, double striking, and evasion, but just falls short in every category. Give me a grave titan, sun titan, invisible stalker, precursor golem, myr battlesphere, wolfir silverheart, or kiln fiend over this any day.
In summary, this needs a very specific deck to work, and just isnt quite good enough to warrant making that deck.
