It can counter an infinite number of spells against a forgetful person. You only have to sacrifice if they remember to ask and you specifically refuse permission.

Errata:
When a spell is cast, counter that spell unless its controller asks your permission to cast that spell. If you refuse permission, sacrifice Common Courtesy and counter that spell.
Well, this is why counter-based control decks are called "Permission" decks.

This is nice, lets you tap out and still counter. Bonus if they forget to ask permission on something.

When you ask nicely to play the spell, and they refuse, simply say "I'm still gonna!" and play a Cancel spell of your own. Do this after you say it, but before the sacrifice step of Common Courtesy.

If your opponent is using an infinite combo loop, you can get a really good rhythm going.

I wish this were legal in any format. There's always that guy who rulesharks "you didn't't respond/said ok..." This seems like a giant middle finger to those people. I'd also like to see this in an enchantment heavy Super-EDH game with like 12 players. See how many people are paying attention.

I feel like this could be a really interesting mechanic in the regular game. You could just make target player choose whether to allow the spell to resolve or to sacrifice the enchantment and counter that spell. Sounds like fun.
