OK, this card is expensive: three black mana is no joke, and four mana for a 2/3 is a lot. However, thinking about the 1994 standard creature costs it is not SO high...
As a Vorthos, I love it! Its plain and simple name and its creepy artwork make this card one of the most flavorful in Magic.
About playability, I think I obvious that for four mana in black there are far better options!

I guess it's now a bit better with planeswalkers. Hit an opponent, and this dude will keep any planeswalker that players gets into play small or even kills it.
High Synergy with Zombie Master. The master gives the fallen evasion, to increase the chances of hitting an opponent, and keeps it in play with regeneration.

It would be cool and useful if it kept damaging them for the rest of the game even after leaving play; but as it is, it's just not very good. If you want to ping your opponent for one every turn, there are much better ways to do it.
