You can get card advantage with this card, but it's not reliable. Going first, if they don't make a play by turn 2, then at end of turn you play this and draw 2. They draw 1 and discard (unless they have an instant).
Another way to get card advantage is once they have no more hand, at which point you need something like Liliana Vess, or any other reusable discard. Simply draw two and give them one, then take their one away (as long as it's not an instant they can play, that is).
Lastly, if you draw two cards and your cards are better, you can get card advantage as a result of holding two spells to their one. For example, let's say it's their end of turn and you both have empty hands except for this card you happen to be holding (which you play). You draw a Counterspell plus a card like Wrath of God or Stroke of Genius, and for the sake of argument, assume they draw a good card as well. Simply Wr... (see all)

The fact that this is an instant means casting it on your opponent's end step gives you a full turn head start to use the extra card before your opponent usually. Howling Mine and Temple Bell don't create card disadvantage, they merely favor the player with a better deck by speeding up the card draw ahead of the land drops. That said, there are decks that win by forcing card draw.
