This card should be able to find use in a deck that runs some discard effects - Megrim will be far more preferable, though.
I also thought about using it in a deck running Underworld Dreams and card drawing effects, but this requires to make your opponent keep more than four cards in hand - which is pretty hard to achieve, and will also raise the possibility that he/she draws a response for the Skullcage. Even if you should be able to do so, e.g. with a Stasis deck, Eboni Owl Netsuke will do a better job then, therefore i guess you can forget about that use.
But aside from that, this card can be useful in general too, since your opponent won't always want to keep at least three cards in his or her hand to prevent the damage since he/she considers playing the cards in hand to be worth more than taking the damage.

It's useful in an artifact deck or a control deck that occasionally needs to kill someone without touching them. It's often difficult to have exactly 3 or 4 cards in your hand, I've found. I've killed people with this and I've been killed by it when I've had it used against me. I think it's underrated.

I think it's more damaging to give in and try to maintain your hand. Consider, for 1 less CMC, you could have Phantom Warrior, which is less conditional way to deal 2 damage a turn.

Run this with a discard deck, so once they have no more cards to discard they're still taking damage.
